Located at 2100 North 49th Street, the 96-bed skilled nursing facility\u202fprovides post-hospital care, short-term\u202frehab\u202fand long-term residential care.\u202f\u202f \u202f<\/p>\n<p>The world\u2019s leading nonprofit focused on heart and brain health, the\u202fAHA\u202fhas invested more than\u202f$4.8 billion in research since its founding.\u202fEvaluation criteria for its new skilled nursing certification \u2013 earned by less than 1% of skilled nursing facilities nationwide, to date \u2013 was designed by heart failure experts.
